Specimen Requirement UPDATE:Vitamin B12

HHLA now requires a purple top tube for Vitamin B12 testing. HHLA's requisition included in the Lab-in-a-Box kit will be updated in time. Test update: Wound Culture

Do you require a Gram stain in addition to a wound culture? Gram stain testing is not automatically performed on every wound culture specimen. Gram stain is recommended with all anaerobic cultures (additional charge). Be sure to mark “Gram stain” in the Microbiology/Cultures section of your HHLA requisition. Please remember to include all ICD-9 codes on every requisition.

Providing HHLA complete billing information saves valuable time and, reduces phone calls and follow-up to your office. Please remember to send the complete yellow card and ICD-9 code with each specimen collected.  

Verbal Test Request Authorization Form

Federal Law (42 CFR 493.1241) requires that a written authorization for verbally ordered clinical laboratory tests be submitted to the laboratory. In addition, When requesting additional tests, you must fax a signed copy of the Verbal Test Request Authorization Form to HHLA (615-771-0335). Click here to download this form.
